All Coaches hold a Clear CRB Check, an FA Coaching Badge,

Emergency First Aid Cert and Child Protection Certificates.

a cheque made payable to 'Skill 7' with your
child's name and venue on the reverse

Send the form and the cheque
to the address on the top of the form
- Please do not send booking forms to any venue

Refunds will only be given if
the course is cancelled by Skill 7 Ltd

Bad behaviour will result in
non-refundable dismissal from the course
